Does someone who succeeds their Will save against a Frightful Caw become immune for 24 hours or do they have to make a Will save every time? Most monster's fear effects have a 24 hour immunity but there is not this caveat given in the Rook description.

I recently ran a group through this module and played the frightful caw as a once per day effect with a 24 hr immunity if you make your save. If you play the caw as an attack Rooks can do more than once per day, e.g. once per round, then the Rooks can quickly cause party members who are not immune to fear to run away. This happens because even a successful Will save still results in the target of a fear spell being shaken. And a shaken character who gets shaken again becomes frightened and then panicked.

That said, I think you've answered the question yourself. It was intended that if a character saved againt the rook's caw attack, he or she would be immune for the balance of a 24-hour period---just as you hinted at. It was an omission on my part to mention that in the text.
That said, I have run the module against more powerful characters and used the attack more frequently....it's an effective balancing tool, if used effectively. I allowed any given rook to use its ability once per combat, for example.
Hope that explanation helps. Keep your eyes out for the 3.5 edition. Joseph Goodman was kind enough to allow me to write a short sequel for the adventure, and I think it adds to the story immensely.
