Chaotic does not mean evil, at least not to me, in this game. Civilization and attempts to order the universe are Lawful, attempts to undermine that order is Chaotic. Cthulhu is Neutral due to his indifference to these struggling forces.

I think the idea is that Cthulu existed prior to the constructions of Lawful and Neutral and is outside of the struggle of order vs. entropy.
At least, that's how I explain it to my players when they ask this question.

No houserules on my end for this. There is a mercurial affect that allows the spellcaster to take any lower results. Maybe offer that to one spell of the players choice?

I roll for their physical stats randomly (1d3, 1 = Str, 2 = Agi, 3 = Sta) when they hit 0 HP.
In my current campaign, I think we had like 6 deaths in three sessions, though none have permanently died.

They do not. A player receives Invoke Patron after casting Patron Bond. Depending on how well they succeed at their bond determines whether or not they get access to the Patron Spells at all.

I would change their hit dice to be an average of the two. Examples:
Cleric + Thief = d7
Cleric + Warrior = d10
Cleric + Wizard = d6
