Does a thief add their attribute bonus, bonus from table 1-9 and level when rolling for a skill check
or
only attribute bonus and bonus from table 1-9

Why would they add their level? Skill check if untrained is d10 + attribute bonus, if trained d20 + attribute bonus. Any kind of class bonuses are added to these.

For Thief skills, applicable attribute bonuses are noted on the table.
