If there's a fight going on in an underground courtyard, how should I determine whether the goblin guard in the room down the hall hears what's happening or not? Are there rules for hearing checks and how far the sound of battle carries (or doesn't)? Or should I just decide if it fits with the story and make it happen or not?

Decide what you think is likely, or assign odds. Or have one of the PCs roll a Luck Check.

Also: are the PCs making any effort to be as quiet as possible in the circumstances? Are their opponents trying to make enough noise to summon allies? Are the goblins down the hall deep in their cups? Are they actively listening for noises beyond their little hidey-hole? Are the acoustics favorable or unfavorable? Are there other noises going on continually (rushing water, etc.)?
It's all about the circumstances.
